Treating exercise oscillatory ventilation in heart failure: the detail that may matter.

چکیده
O ver the last 15 yrs, cardiologists have had to face a progressive recognition that abnormalities in the ventilatory response to exercise are relevant features of the heart failure syndrome [1]. Prior pathophysiology studies have defined the mechanistic role of the lung (impaired perfusion and increased minute ventilation (V9E)) [2] and of the periphery (increased chemo/metaboreflex sensitivity) [3] as determinants of the inefficient ventilatory response and early exercise termination.

Relation between oscillatory ventilation at rest before cardiopulmonary exercise testing and prognosis in patients with left ventricular dysfunction.
BACKGROUND Although nocturnal Cheyne-Stokes respiration alternating between hyperpnea and hypopnea has been considered a sign of severe heart failure, the clinical status of cardiac patients who exhibit oscillatory ventilation during wakefulness has not been clarified. This study was carried out to determine the relation between oscillatory ventilation during wakefulness and exercise capacity i...

Despite improvement of mechanical ventilation devices for infants, lung injury still causes morbidity and mortality in many cases. To determine the effect of high frequency oscillatory ventilation (HFOV) in infants with severe respiratory failure, 10 neonates were evaluated prospectively. Mean gestational age of these patients was 35 weeks and alveolar-arterial oxygen gradient (A-aDo2) and the ...
متن کاملExercise training reverses exertional oscillatory ventilation in heart failure patients.
Exertional oscillatory ventilation (EOV) is an ominous prognostic sign in chronic heart failure (CHF), but little is known about the success of specific therapeutic interventions. Our aim was to study the impact of an exercise training on exercise capacity and cardiopulmonary adaptation in stable CHF patients with left ventricular systolic dysfunction and EOV. 96 stable CHF patients with EOV we...
متن کاملHeart Failure Exercise Oscillatory Ventilation in Systolic Heart Failure An Indicator of Impaired Hemodynamic Response to Exercise
Background—Exercise oscillatory ventilation (EOV) is a noninvasive parameter that potently predicts outcomes in systolic heart failure (HF). However, mechanistic insights into EOV have been limited by the absence of studies relating EOV to invasive hemodynamic measurements and blood gases performed during exercise. Methods and Results—Fifty-six patients with systolic HF (mean SEM age, 59 2 year...
